While it sucks and in no way defending these actions many sites including twitch, youtube, etc typically rely on advertisers. So deprioritizing sex and violence is typically the caused due to advertisers dictating what material they want associated with their brands. Coke probably doesn't want to be associated with a person getting double teamed if they are aiming for a "family brand". eXtwitter is probably trying to appease advertisers since they screwed the pooch ever since Elon took control and they lost a ton of negotiation power. If websites thought they can make $$$ being sex worker friendly they would do it but typically the ads for adult sites have terrible payouts. This is the dangers of a "free" internet.

There are some things only achievable via massive corporations. You aren't going to be able to copy a youtube or twitch on a fediverse style, well technically possible but very ill advised since storing full video is pricey and the bandwidth requirements will kill the host in costs.
